Students Applying for Ohio Board Licensure:

How to apply for Licensure from the Ohio Board of Speech Pathology

Licensure from the Ohio Board of Speech Pathology is necessary before you apply for licensure from the Ohio Department of Education (Must attach the board licensure to your application to ODE)

The following are the instructions for applying for an Ohio license.

On the web, go to www.ohio.gov
Click on Government on the very right at the top of the page.
Click on Agencies, Departments and Board and Commissions on the right.
Scroll down to Speech-Language Pathology and Audiology and click on that.
Click on Forms and Applications.
Click on General Application Forms.
Under speech-language pathology options, click on online application or application for licensure.  (It may be easier to do the hard copy since you have to send a picture.)

Fill out the application.  DO NOT indicate any previous employment.  They are asking only about speech-language pathology.

You must send the following:

  • Completed application with picture
  • Fees
  • Your PRAXIS scores sent directly from ETS. 

ALL information must be at the board of Speech Pathology 7 days before the 1st or 15th of the month, when they do licensing.
